Our network of nearly 200-member agencies across the region, covers virtually every corner of Northwest Ohio, ensuring that no one in our community goes hungry. Member Agencies include food pantries, soup kitchens, emergency shelters, rehabilitation centers, special programs for the elderly, group homes, holiday food basket programs and non-profit day care centers.
To initiate the application process, kindly complete the application form and submit it along with the following required documents:
Proof of existence for a minimum of 6 months
Current list of board members and/or trustees
Copy of your IRS Letter or equivalent documentation
Signed copy of the food bank guidelines
Signed Member Agency Letter of Agreement
Signed Disclaimer Form
*Private foundations (even though they are tax-exempt under IRS Section 501(c)(3) of the Code) and foster parent associations are not eligible to become Member Agencies.
